Job Description:

 This member of the multidisciplinary team has the responsibility to carry out rehabilitation, support functions, and assist in the recovery process; provides substance abuse services, educational support, and consultation to families; provides crisis intervention services under clinical supervision of staff with professional degrees. 
 
Responsibilities:
  •  Be a member of the individual treatment team for an assigned number of consumers. 
  •  Participate in treatment planning and implementation; recovery support services; physical health and dental health monitoring; education and employment assistance; social development and functioning; activities of daily living (e.g., self care, living situation, nutrition, money management); and family structure and relationships.  
  • Consult with community agencies and families to maintain coordination in the recovery process. 
  •  Document consumer progress to maintain a permanent record of consumer activity, according to established methods, policies, and procedures.  
  • Participate in organization meetings, recovery planning, and review meetings. 
  •  Participate in providing treatment and substance use services. 
  • Must have a reliable insured vehicle

Support 
 
Provide practical help and support, advocacy, coordination, side-by-side individual support, problem solving, direct assistance, training, and supervision to help consumers obtain the necessities of daily living, including medical and dental health care; legal and advocacy services; financial support such as entitlements (e.g., SSI, SSDI, and Veterans Benefits), or housing subsidies (e.g., HUD Section 8); money-management services (e.g., payee ships); and transportation.  
 
Education, Experience, and Knowledge Required 
 
The Rehabilitation Specialist must have a Bachelor’s degree in the field of behavioral sciences, and have experience working with adults with severe and persistent mental illnesses, or individuals with similar human-services needs. May also have related training (e.g., certified occupational therapy assistant, home health care aide), or work experience (e.g., teaching) and life experience. Must have a strong commitment to the right and ability of each person with severe and persistent mental illnesses to live in normal community residencies; work in market jobs; and have access to helpful, adequate, competent, and continuous supports and services.
