Job Summary:
General knowledge of payor industry (Medicare, Medicaid, and Commercial insurance regulations), resource management, and evidenced based clinical practice is essential. Theoretical knowledge of the nursing and case management processes. Analytical skills necessary to assess patient's discharge needs and develop solutions to problems concerning patients' mental status or physical well-being to ensure optimal patient outcomes. Advanced communication and interpersonal skills with all levels of internal and external customers including but not limited to medical staff, patients and families, clinical personnel, support and technical staff, outside agencies, and members of the community. Other skills needed include excellent written/verbal communication skills, critical thinking, creative problem solving, proficient organization and planning. Basic computer skills. Must be self-directed with ability to tolerate frequent interruptions and a demanding workload. Other duties as assigned.

EXPERIENCE DESCRIPTION:

Minimum of three years hospital acute care nursing experience required. Minimum of 5 years of hospital acute care experience preferred. Prior case management experience preferred.

EDUCATION DESCRIPTION:

Professional knowledge of nursing theory and practice at a level normally acquired through completion of a program at an accredited School of Nursing. Bachelor's degree in Nursing is preferred.

SPECIAL SKILLS DESCRIPTION:

Analytical skills necessary to assess patient's discharge needs and develop solutions to problems concerning patients' mental status or physical well-being to ensure optimal patient outcomes. Interpersonal skills necessary to instruct and maintain effective contacts with a variety of hospital personnel.

LICENSURE DESCRIPTION:

Requires current state of Wisconsin Registered Nurse License or a Multi-state Nursing License from a participating state in the NLC (Nurse Licensure Compact). Accredited Case Manager (ACM) or Certified Case Manager (CCM) certification preferred.
