100% FTE, Monday - Friday, day shift. No weekends or holidays. Hours may vary based on the operational needs of the department. You will work at American Family Children's Hospital in Madison, WI.

We are seeking a Registered Nurse (RN) Navigator to:

  • Bring your experience in pediatrics, pediatric rehabilitation and/or pediatric developmental disabilities.
  • Coordinate admissions and assist with coordination of discharges.
  • Coordinate family teaching about new rehab conditions, ensure smooth process for family from pre-admission through return to home.
  • Collaborate with medical team and/or observe therapy sessions as needed to help determine patient/family education needs.
  • Administer and review discharge surveys, lead QI initiatives, and assist with requirements for CARF accreditation.

Education:

Minimum - Bachelor of Science - Nursing degree (BSN).

Preferred - Masters degree in Nursing, Social Work, Public Health, Healthcare Administration, or related field.

Work Experience:

Minimum - Three (3) years of relevant clinical nursing experience.

Preferred - One (1) year of relevant experience in the specific patient population being supported.

Licenses and Certifications:

Minimum - Registered Nurse license in the State of Wisconsin. Basic Life Support/CPR.

Preferred - Applicable Oncology Certification based on patient population.
