Position Summary:

Utilizing the case management process, the Admission Case Manager (ACM) coordinates transitional care for the children/families on an assigned unit. Responsible for level of care assessment, documentation, and corrections to patient class in the electronic health record (EHR). The ACM collaborates with providers to assure all admission status orders are present and accurate. In addition, based on the needs of the department also performs the functions of a unit based Hospital Case Manager (HCM).

Position Requirements:

  • Knowledge of patient care, case management theory and practice preferred.
  • Registered Nurse (BSN preferred) with a current license to practice professional nursing in the State of WI.
  • Minimum of 2-3 years of case management experience preferred. Certification as a case manager preferred.
