Work Schedule:

This is a full time, 1.0 FTE day shift position. Hours will be primarily Mondays - Fridays, 8:30AM - 5:00PM, no weekends required. Hours may vary based on the operational needs of the department.

The UW Health Compass Clinics are low barrier medical clinics that enable individuals to receive immediate access to Medications for Opioid Use Disorder (MOUD). Create pathways for patients to seek resources within the UW Health and UPH Addiction and Primary Care system of care.

We are seeking an Ambulatory Social Worker to:

  • Evaluate complex patient and family situations which have legal, ethical and psychosocial components.
  • Collaboratively develops patient goals, monitors progress, addresses barriers to goal achievement and evaluates outcomes of individualized care plans.
  • Engage patient/family in the treatment and decision-making process with respect to patient rights, principles of confidentiality, respect for patient privacy, and right to self-determination.
  • Utilizes crisis intervention, conflict resolution, and mediation to address patient/family concerns.

Education:

Minimum - Master's Degree in Social Work

Work Experience:

Preferred - Two (2) years prior hospital based social work experience

Licenses and Certifications:

Minimum - Licensed in the state of Wisconsin with one of the following within 90 days of hire:

  • Certified Advanced Practice Social Worker (CAPSW)
  • Certified Independent Social Worker (CISW)
  • Licensed Clinical Social Worker (LCSW)
