Work Schedule:

100% FTE, days. This position will work primarily remote, with some travel to behavioral health clinics within the Madison area.

We are seeking a Social Services Complex Case Manager to:

  • Provide case management for patients with complex psychosocial needs and facilitate solutions to the resource needs, behavioral health needs, and psychosocial/environmental issues of patients and families
  • Collaborate with internal and external multi-disciplinary teams
  • Engage patients with the health system and assist team members, patient, and family in overcoming barriers to do so

Education:

Minimum - Master's degree in Psychology, Social Work, Counseling, or other Behavioral Science related degree

Work Experience:

Minimum - Two (2) years of case management experience

Preferred - Case management experience in a behavioral health setting

Licenses and Certifications:

Minimum - None

Preferred -

  • Valid driver's license and transportation
  • Case Management Certification
