Per Diem Registered Nurses must commit to work a minimum of seventy-two (72) hours per six (6) week scheduling period as follows: The Per Diem may be required to work evening and/or night shifts, weekend hours, and holiday hours. This position is anticipated to work a 40% FTE. You will be working at University Hospital and East Madison Hospital in Madison, WI.

We are seeking a Per Diem Registered Nurse (RN) Transitional Care Case Manager to:
At UW Health, you will have:
Education:
Minimum - Bachelor's degree in Nursing.
Preferred - Master's degree in Nursing or Health Care related field.
Work Experience:
Minimum - Three (3) years relevant clinical nursing experience. Equivalent combination of education and experience will be considered.
Preferred - Surgical, trauma, and intensive care experience.
Licenses and Certifications:
Minimum - RN licensed in the state of Wisconsin. CPR certification.
Preferred - ACMA certification as a case manager.
