Job Description
JOB TITLE: Crisis Stabilizer - CCM - TCM                      

FLSA STATUS: Hourly / Overtime eligible

JOB LOCATION: Social Services - Adult Programs                                 

PRIMARY FUNCTION: Provides stabilization and support of the client’s wellbeing and support of the case manager, peer specialist, and clinician to meet the client’s crisis/safety plan. 

PRIMARY DUTIES:

  1. Provide help and assistance with the delivery of all aspects of individual support to the client such as transporting client to appointments,  assist in developing a comprehensive safety plan for use during times of crisis, meet with the client when  admitted to hospital, be available to meet with client within 24 hours of receiving the referral, assist in linking client to resources in the community and work with the crisis team to assist through the recovery process (family support and community based),  and assist in the development of a Crisis Plan, to guide the consumer through their recovery. E
  2. Provide supportive services with a person centered trauma informed focus. E
  3. Communicate with applicable stakeholders as appropriate in a timely manner after each crisis incident. E
  4. Prepare, complete, and submit required items by due dates to include, but not limited to evaluation of services provided, documentation of all interventions, preventions, and stabilization contacts. E
  5. Attend meetings and professional development activities, as required. E
  6. Establish and maintain good communication, collaboration, and cooperation with all clients, parents, co-workers, etc. E
  7. Follow all duties of a mandated reporter. E
  8. Perform other related duties, as assigned. NE

WORKING CONDITIONS:

Work is generally performed in field and/or office environments.  Local travel required; some state-wide travel may be needed. 

REPORTS TO: Targeted Case Manager Supervisor or designee


MINIMUM QUALIFICATIONS: 
 
  1. Bachelor’s degree from an accredited college or university in Social Work or closely related field.
  2. One year of experience working with adults experiencing mental health and/or substance use.
  3. Bilingual (Spanish and English), spoken and written. (Desirable)
  4. Neat orderly person with great organizational skills.
  5. Must be a “People Person” with a great disposition and outlook on life, who enjoys working with others, is upbeat and pleasant to work with and is a good listener.
  6. Knowledge and experience with Microsoft Office suite.
  7. Must be willing and able to work flexible hours as needed.
  8. Excellent communication and documentation skills, written and verbal
  9. Must pass all background checks.
