About Us:
The Clement J. Zablocki VA Medical Center in Milwaukee, Wisconsin (http://Milwaukee.va.gov) is a federal facility located on 245 acres on the western edge of Milwaukee and part of VA Integrated Services Network 12 (VISN 12), which includes facilities in Iron Mountain, Michigan; Tomah and Madison, Wisconsin, and North Chicago, Hines, and Chicago in Illinois. The Medical Center delivers primary, secondary, and tertiary medical care in 196 hospital beds, and provides more than 857,000 visits annually through an extensive outpatient program. The nursing home care unit of 113 beds offers geriatric programming and the 150 domiciliary beds are the focus of programs in Substance Abuse Rehabilitation, Psychiatric Rehabilitation and Post Traumatic Stress Disorder care.
Duties Include:
Assist patients/residents in the performance of activities of daily living, including bathing, toileting, and providing patients/residents with nutritional support.
Perform basic restorative care including ambulation, and assisting with transfers. Appropriately use assistive devices for patient/resident transfer and ambulation.
Obtain vital signs and specimen collections in accordance with established guidelines.
Recognize and report changes in patient/resident condition, vital signs, input/output measures, and weights.
Recognize, report, and address any safety hazards, errors, falls, injuries, equipment failure, or concerns with staff/patient behavior.
Provide continuous protective observation in accordance with hospital guidelines.
Understand basic body mechanics to ensure safe movement, transport, and positioning of patients/residents.
Adhere to infection control policies and guidelines. Use universal precautions when performing patient/resident care.
Activate emergency response system, and perform BLS measures until appropriate assistance arrives.
Appropriately document/record observations and actions in the patient's/resident's medical record and/or on unit worksheets.
Respond to patient/resident call lights or requests for patient/resident assistance in a timely manner. Consistently address patient/resident, family, and staff with courtesy and respect.
Demonstrate effective communication techniques for patients/residents with dementia. Offer personalized care to patient/residents with dementia.
Provide pre and post-procedural care including the reinforcement of patient/resident and family teaching provided by LPN/RN/MD.
