Job Description

CLASS SUMMARY:

This position is responsible for providing a variety of services to residents to assist them in gaining self-sufficiency. Responsibilities may include assessing needs; identifying available services; and providing ongoing support to clients.

TYPICAL CLASS ESSENTIAL DUTIES: (These duties are a representative sample; position assignments may vary.)

Conducts interviews and/or assessments to determine individual needs, job interests, qualifications, and eligibility for services.

Provides guidance and mentorship to individuals, including: assisting them in creating individualized self-sufficiency plans and goals; providing continual feedback and guidance on progress towards goals ; assisting with job search tasks; and assisting residents in accessing community-based resources and services.

Build partnerships with employers and service providers in the community and work with local service providers to ensure that program participants are linked to the supportive services they need to achieve self-sufficiency, including but not limited to:  job training; financial education; job placement; child care; transportation; homeownership; etc.

Establishes relationships with and communicates with clients, caseworkers, community partners and staff to promote the self-sufficiency program. 

Maintains client information in case management and other databases.

Sets monthly goals and prepares reports on program outcomes.

Monitors individual’s  compliance with their Family Self Sufficiency (FSS) Contract of Participation and assists in recommending changes in goals and activities, terminations from the program, early escrow disbursements, or  final disbursements from FSS escrow accounts. 

May also provide guidance and assistance to eligible families regarding participation in the Section 8 (y) homeownership program.

Performs other duties of a similar nature or level.

TRAINING AND EXPERIENCE (positions in this class typically require):
Associate’s degree in Human Services or related field; 2-4 years of related experience; or, an equivalent combination of education and experience sufficient to successfully perform the essential duties of the job such as those listed above.

LICENSING REQUIREMENTS (positions in this class typically require):

Wisconsin Driver’s License

  • Research methods;
  • Case management principles and practices;
  • Applicable contract requirements.

  • Utilizing communication and interpersonal skills as applied to interaction with coworkers, supervisor, the general public, and others sufficient to exchange or convey information and to receive work direction.
PHYSICAL REQUIREMENTS:
Positions in this class typically require: reaching, standing, walking, fingering, grasping, feeling, talking, hearing, seeing, and repetitive motions.

Sedentary Work: Exerting up to 10 pounds of force occasionally and/or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects, including the human body.  Sedentary work involves sitting most of the time. Walking and standing are required only occasionally.
