Job Summary and Purpose:

Coordinates and administers Physical Therapy care to patients which involves assessment of the patient's physical and psychosocial needs; planning development and implementation of a multidisciplinary plan of care; and evaluation of the patient's response to care.

Qualifications:

Graduate from an accredited program for Physical Therapy. Current licensure as a physical therapist in the State of Wisconsin. CPR certification or ability to obtain within 3 months. No physical therapy work experience required.
