We are currently seeking candidate's interested in Full-Time, Part-Time and Casual/Weekend/Pick-Up employment opportunities for all shift options. As one of the country's industry leaders, we provide exceptional senior care and services throughout Wisconsin. Heritage Senior Living's goals are to enhance residents' quality of life and to continue its growth and success. Our plan is to open one to two new senior communities per year and develop centers of excellence that provide comfort, care, quality, and specialized dementia services.Caregivers and CNAs are responsible for providing high-quality care to residents per their individualized service plan. Caregivers and CNAs attend to our residents' individual needs, assist with activities of daily living, coordinate work within the department, promptly report pertinent information to the immediate supervisor, and maintain the comfort, privacy, and dignity of our residents.Essential Duties and ResponsibilitiesProvide quality care to residents in an environment that promotes their rights, dignity, freedom of choice, and individualityProvide individualized attention, which encourages each resident's ability to maintain or attain the highest practical physical, mental, and psychosocial well-beingRead and understand individualized service plans (ISPs) for each resident and provide support to the residents per their service planContribute to the service planning process by providing information and observations of the residents' needs and preferencesAttend to residents' individual needs, including assistance with grooming, bathing, oral hygiene, eating, incontinence care, toileting, colostomy care, prosthetic appliances, transferring, ambulating, range of motion, communicating or other care needs as identified in residents' ISPsObserve residents for changes in condition or behavior and promptly report these changes to the supervisorPromptly answer and courteously respond to residents' pages and alarmsMaintain residents' comfort, privacy, and dignity in the delivery of servicesInteract with residents in a warm, kind, and caring mannerComplete records documenting the care provided or other information in keeping with department policiesFully understand all aspects of residents' rights, including the right to be free of restraints, abuse, neglect, misappropriation of property, and financial exploitationResponsible for promptly reporting incidents or evidence of resident abuse or violations of residents' rightsAssist in maintaining a safe, neat, clean environment and report environmental deficiencies such as lighting or equipment problems to the supervisorProtect residents' personal belongings, including eyeglasses, dentures, hearing aids, furnishings, jewelry, clothing, medications, and moneyImmediately report lost, missing, or stolen items per established policies and procedures and participate in efforts to locate itemsPerform various tasks as assigned, including checking vital signs, weighing residents, applying creams/ointments, collecting specimens, laundry, passing medications in a timely manner, etc.Lift, move, and transport residents using proper body mechanics and lifting devicesCommunicate and interact effectively and tactfully with the residents, visitors, families, peers, and supervisorsAssist and escort residents for appointments, such as at the beauty shop, or to lifestyle programsPractice careful and efficient use of supplies and linens and follow established procedures for resident charge itemsPerform all job responsibilities in accordance with prescribed safety and infection control procedures, including thorough hand washing, use of disposable gloves where indicated, and proper disposal of soiled materialsComplete rounds and scheduled wellness checks to ensure the well-being of residentsPromote a homelike environment for residentsParticipate in daily care focus meetingsParticipate in weekly care coordination meetingsAttend trainings as scheduled, including flash trainings, Relias, etc.Administer medications (after delegation by RN)Education/QualificationsHigh school diploma, general education degree (GED) or 1-3 months related experience or training (preferred)If a CNA, must provide a copy of certificateAbility to speak, write, and read EnglishCaring and compassionate attitude with a true concern for people requiredStrong customer service orientationAbility to work collaboratively as part of a teamAbility to multitaskMust submit and pass all applicable pre-employment and annual tests and meet all training and other applicable state and federal regulationsCompetency and ability to utilize a floor lift, sit-to-stand and friction-reducing devicesEssential FunctionsTo perform this job successfully, an individual must be able to perform each essential duty.
