Job Description
Heartland Hospice a part of HCR ManorCare/ProMedica that provides a range of services, including skilled nursing care, assisted living, post-acute medical and rehabilitation care, hospice care, home health care and rehabilitation therapy is currently searching for a full-time Weekend Patient Care Coordinator to support the triage hospice and home health team. This full-time role will work weekends, 430pm Friday-Monday at 8am. The Patient Care Coordinator is responsible for the work flow coordination within Home Care Home Base. In addition, PCC must be able to do an assessment and evaluation of patients. The Patient Care Coordinator assures compliance with agency standards and state and federal laws. This rolel will also involve approving and ordering DME supplies as needed, reviewing and adjusting schedules, answering calls and triaging, data entry of visits and other information, managing staffing needs, reviewing logs, facilitating stand up calls, etc. We prefer a minimum of 2-3 years of hospice experience, excellent verbal and written communication skills, strong ability to manage competing priorities, and a calming presence. Previous management or supervisory experience preferred.
