Description: Heartland Home Health Care a part of HCR ManorCare/ProMedica thatprovides a range of services, including skilled nursing care, assisted living, post-acute medical and rehabilitation care, hospice care, home health care and rehabilitation therapy is currently searching for a full-time, 32 hour/week Physical Therapist to support their home health patients. At least one year or more work experience in physical therapy in a multidisciplinary setting is preferred. Excellent communication skills, and ability to develop and maintain rapport with clients and families.Educational Requirements Bachelor's or Master's degree in physical therapy from an accredited institution.
