Under minimal direction, to perform social work services at a comprehensive behavioral health facility serving children, adolescents and adults in a continuum of care providing emergency, outpatient, day hospital, inpatient, and aftercare services. Position Duties: Make psycho-social and AODA assessments. Develop and execute discharge planning. Act as a liaison with community resources to facilitate aftercare services and placements Function as a member of a mental health team in diagnostic formulations and implementation of treatment plans with patients and families Function as the team's Treatment Plan Coordinator in developing and recording treatment plans; participate in multi-disciplinary team staffing; meet productivity goals established by the Department Present policies on Advanced Directives to patients and families. Provide education and information on mental illness and community resources to families of patients. Confer with community agencies, schools and courts; serve as a consultant to the courts if requested. Participate in staff development and quality assurance activities. Contribute to planning and evaluation efforts in the development of community health services; participate in community consultation and education program. Maintain records and prepare reports according to agency and funding source guidelines. Perform other duties as assigned. |
Minimum Qualifications |
Knowledge, Skills and Abilities: Knowledge of and skill in the application of ethics, theories, methods, practices, treatments and techniques of psychiatric social work; oral and written communication skills; knowledge of health services providing care to children, adolescents and adults; knowledge of community resources in the area of health and social services and the ability to utilize them effectively; knowledge of mental disorders and diseases; knowledge of laws governing mental commitments; skill in interviewing patients and family members and accurately assessing their needs; ability to work as a member of a mental health or health care team; ability to work with people of all social, economic and educational levels; ability to develop effective interpersonal relationships; ability to work effectively and harmoniously with staff, patients, families, and agencies.
