Lutheran Social Services of WI & Upper MI in collaboration with Racine County is currently seeking a Case Manager to join the Community Support Program. Community Support Programs are for adults living with a serious and persistent mental illness. CSP provides coordinated professional care and treatment in the community that includes a broad range of services to meet an individual's unique personal needs, reduce symptoms, and promote recovery. The program is intended to assist consumers with their individual needs from minimal to intensive, or a level that might otherwise require care in a hospital setting.
This opportunity requires staff to visit clients in home and travel throughout Racine County. The position is M-F, generally 8 AM - 5 PM.
This position involves the care and treatment of adults diagnosed with a chronic mental illness. Position delivers psychosocial rehabilitative services in accordance with a CSP plan of care. Duties include development and implementation of the treatment plan, which will include the necessary follow up, case management and interventions, as required. Works with other case managers, consulting psychiatrist, front desk staff, and director of quality assurance, consumers and managers.

EDUCATION AND/OR EXPERIENCE:

Must have a valid driver's license and have reliable transportation to perform the essential duties of the role; a motor vehicle check (MVR) with a satisfactory driving record per the LSS Driver Safety Procedure is required, and ability to meet LSS auto insurance requirements.
TRAVEL: Ability to travel on day trips as required. Some overnight travel may be required.
