Community Care is a growing non-profit managed care provider dedicated to enhancing the quality of life for adults with disabilities and frail elders by helping them continue to live in the community as independently as possible for as long as possible.

Full-time RN Care Manager are needed to assist the interdisciplinary team in identification and monitoring of clinical issues during assessment, service plan development, ongoing care coordination, quality assurance and ongoing monitoring of services to program clients. An RN degree or diploma, current state of Wisconsin Registered Nurse license, and a minimum of two years recent experience with older adults or persons with physical disabilities in a hospital, homecare setting, or nursing home experience required. BSN and community nursing experience are preferred. Car necessary for routine home visits.
