Position Description
Westside Healthcare Association, Inc. is a non-profit corporation, which administers two primary health care clinics, is seeking a dynamic professional to join our Case Management Team.
GENERAL RESPONSIBILITIES INCLUDE:
Collaborate with physicians, dentists and other health care professionals to evaluate patients' medical or physical condition and to assess client needs.
Refer patient or family to community resources to assist in recovery from mental or physical illness and to provide access to services such as medical and financial assistance.
Counsel clients and patients in individual and group sessions to help them overcome dependencies, recover from illness, and adjust to life.
Organize support groups or counsel family members to assist them in understanding, dealing with, and supporting the patient.
Advocate for patients to resolve crises.
Identify environmental impediments to patient progress through interviews and review of patient records.
Utilize consultation data and social work experience to plan and coordinate patient care and rehabilitation, following through to ensure service efficacy.
Modify treatment plans to comply with changes in patient, status.
Monitor, evaluate, and record patient progress according to measurable goals described in treatment and care plan.
Develop or advise on social policy and assist in community development.
Plan and conduct programs to combat social problems, prevent substance abuse, or improve community health and counseling services.
MINIMUM REQUIREMENTS:
Master's Degree in Social Work or related field preferred
Two years post degree experience
State of Wisconsin Social Work certification LCSW required and maintained
Excellent oral and written communication skills.
Working knowledge of computers and office application software.
Ability to relate well to people from diverse ethnic and cultural backgrounds.
Ability to work independently
Bilingual Spanish/English preferred
Valid driver's license and insurance required
