Position Description

Westside Healthcare Association, Inc. is a non-profit corporation, which administers two primary health care clinics, is seeking a dynamic professional to join our Case Management Team.

GENERAL RESPONSIBILITIES INCLUDE:
  • Collaborate with physicians, dentists and other health care professionals to evaluate patients' medical or physical condition and to assess client needs.

  • Refer patient or family to community resources to assist in recovery from mental or physical illness and to provide access to services such as medical and financial assistance.

  • Counsel clients and patients in individual and group sessions to help them overcome dependencies, recover from illness, and adjust to life.

  • Organize support groups or counsel family members to assist them in understanding, dealing with, and supporting the patient.

  • Advocate for patients to resolve crises.

  • Identify environmental impediments to patient progress through interviews and review of patient records.

  • Utilize consultation data and social work experience to plan and coordinate patient care and rehabilitation, following through to ensure service efficacy.

  • Modify treatment plans to comply with changes in patient, status.

  • Monitor, evaluate, and record patient progress according to measurable goals described in treatment and care plan.

  • Develop or advise on social policy and assist in community development.

  • Plan and conduct programs to combat social problems, prevent substance abuse, or improve community health and counseling services.

    MINIMUM REQUIREMENTS:
  • Master's Degree in Social Work or related field preferred
  • Two years post degree experience
  • State of Wisconsin Social Work certification LCSW required and maintained
  • Excellent oral and written communication skills.
  • Working knowledge of computers and office application software.
  • Ability to relate well to people from diverse ethnic and cultural backgrounds.
  • Ability to work independently
  • Bilingual Spanish/English preferred
  • Valid driver's license and insurance required
