Position Description

St. Clare Hospital seeks qualified candidate to work Part time, 64 hrs/2weeks in Baraboo WI located about 45 miles NW of Madison WI. Must be a WI Licensed Physical Therapist current in BLS certification. Previous experience preferred but not required.

Provides direct patient care including various exercise programs, cryotherapy, electrical stimulation, elec-tric modalities, muscle re- education, heat applications, hydrotherapy, gait training, traction, massage, man-ual techniques, wound care, transfer training, education and home instruction. Provides total patient care including evaluation, treatment plan, establishing treatment goals with an estimate of time to complete goals, documentation of all relevant information, and communication with patient, other personnel and physicians.

Contact with third party payors as required for each patient. Provide patient care activities in a variety of settings including inpatient, outpatient, extended care facility, home health and industrial set-tings. Conducts and participates in education and training of self, patients, care providers, other health care professionals and the community. Participates in hospital and departmental functions other than patient care, including PI, educational programs, staff meetings, budget preparation, hospital committees and other as requested. Must show an understanding of third party reimbursement and JCAHO standards for the re-habilitation department. Encouraged to be a member of the state and national PT association.
