REGISTERED NURSE - CENTRALIZED SERVICES

This is an excellent opportunity for a Centralized Service-Registered Nurse (24-40 hrs/wk, benefits eligible).

PURPOSE of POSITION:
Under the direction of the Centralized Services Manager-RN and Director of Centralized Services, the Centralized Services-RN will be responsible for providing all aspects of medical follow-up to patients related to lab results, care management plans, and phone triage.

- Experience in electronic health records (EHR), Office 365, and Microsoft Office Products.
- Accuracy and attention to detail are essential.
- Ability to read and implement written and verbal instructions.
- Ability to work independently and as a team member.
- Ability to handle sensitive information in a confidential manner.
- Demonstrated ability to manage health-related emergency conditions including independent decision-making.
- Bilingual Spanish/English skills desirable.
- CPR certifiable.

EDUCATION and/or EXPERIENCE:

- Must have a current license as a Registered Nurse by the State of Wisconsin
- A minimum of one (1) year of nursing experience is preferred.

PHYSICAL REQUIREMENTS:

Physical demands of the position necessitate hearing, talking, handling and grasping regularly. May require lifting up to 30 pounds. Specific vision conditions include close vision reading. Requires the ability to operate a computer, telephone, fax, copy machine and adding machine or calculator. Requires the ability to operate medical equipment.
WORKING CONDITIONS:

The position is generally performed in a medical office environment but involves frequent interruptions. The noise level is generally low to quiet. The work environment characteristics are representative of those an employee encounters while performing the essential functions of this position. Reasonable accommodations, however, may be made to enable individuals with disabilities to perform said essential functions.
Work environment involves exposure to potentially dangerous materials and situations that require following extensive safety precautions and may include the use of protective equipment. Will work with blood or blood-borne pathogens and will require OSHA training.
This position requires occasional traveling, therefore a valid driver's license and auto insurance in accordance with agency liability standards may be required. Must have transportation.

Evening and/or weekend hours may be necessary.

ESSENTIAL DUTIES and RESPONSIBILITIES:
  • Daily review of Provider Approval Queues (PAQ) to include accepting and signing off of allnormal and abnormal laboratory results defined in PPWI protocols, policies and procedures.
  • Responsible for creating patient-related follow-up plans per standing orders for normal and abnormal test results and referrals, task all others, place alerts and start notifications as needed and maintain next action date report system.
  • Provide medically accurate education for all laboratory & medical results as assigned in Centralized Services and accurately document in the medical record/follow-up plan.
  • Accept and manage tasks, make patient contact attempts and provide education materials as required per protocol.
  • Seek consultations as necessary from affiliate clinicians and Chief Medical Officer.
  • Generate referrals for patient care.
  • Assess, plan and administer health care which is individualized to the patient and consistent with the policies and protocols of PPFA & PPWI through patient phone calls.
  • Triage urgent and non-urgent abortion and early pregnancy complication phone calls and manage home pregnancy test medication abortion follow-up.
  • Respond to medication refill requests and prescription issues from patients and pharmaciesand manage prior authorization requests.
  • Run daily reports from electronic health records to determine next action steps in patient follow-up.
  • Review medical records from external providers related to patient's follow-up care or plan.
  • Answer and return phone calls when logged in, be available for transfers from patient call center, check voicemail throughout the day, return appropriate calls and document patient information from calls.
  • Meet metrics related to volume and time for reminder letters and contact attempts.
  • Maintain ongoing and appropriate communication with all staff in Centralized Services as well as the Patient Services team.
  • Participate in agency quality assurance program. Under the direction of the Quality and Risk Manager, implement overall quality assurance activities including audits.
  • Assist with on-call services according to agency protocols.
  • Complete other duties as assigned.
