Work Schedule:

100% FTE, Day/Evening, 7:00am - 3:30pm/8:00am - 4:30pm, with rotating evening, 1:30pm - 10:00pm, and every 4th weekend. Hours may vary based on department needs. You will work at University Hospital in Madison, WI.

We are seeking a Clinical Pharmacist - Transplant to:

  • Provide medication therapy management services as part of the interdisciplinary transplant team to patients on inpatient and ambulatory services to assure that drug therapy is safe, effective, efficient, and cost-effective.
  • Perform both clinical and distributive tasks necessary to ensure that patients' medication therapy is safe, effective, timely, and efficient.
  • Provide training and education to Pharmacy Residents and PharmD students through practical and didactic experiences.
  • Provide patient care to a diverse population including kidney, liver, pancreas, heart and lung transplant medical and surgical patients.

Education:

Minimum - Bachelor's degree in Pharmacy or a PharmD degree

Preferred - Completion of an ASHP Accredited PGY2 Transplant Residency training program

Work Experience:

Minimum - One year of clinical patient care and orders management experience

Preferred - Three years of institutional experience (hospital or integrated health system) or completion of an ASHP accredited PGY2 Transplant Residency training program

Licenses & Certifications:

Minimum - Licensed to practice pharmacy in the State of Wisconsin or must obtain within 90 days of hire date

Preferred - Certification through the appropriate certifying program (Board of Pharmacy Specialties).
