Shift: Shift 1

Monday through Friday post liver transplant coordinator position. WFH is an option after successful orientation. will only be allowed 2 days WFH after completion of successful orientation. hours typically 8-430 but can be flexed given this a salaried/exempt position.

Job Summary:

This Transplant Coordinator position provides primary clinical management and coordination of care for all post-transplant adult kidney, pancreas, and liver transplant patients. The Transplant Coordinator interacts with all necessary disciplines to coordinate patient services across the continuum of care. The person in this position must have exemplary customer service skills. Other duties as assigned.

Other information:

EXPERIENCE DESCRIPTION: Minimum of 3 years of acute care or dialysis experience is required.
EXPERIENCE PREFERRED: Experience relevant to Nephrology/Transplant sub-specialties is preferred
EDUCATION DESCRIPTION: Associate's degree in Nursing with a minimum of 10 years of relevant experience is required or Bachelor's degree in Nursing with a minimum of 3 years of relevant experience is required.
EDUCATION PREFERRED: Bachelor's degree in Nursing is preferred.
TRAINING DESCRIPTION: None. TRAINING PREFERRED: None. SPECIAL SKILLS DESCRIPTION: Analytical skills necessary in order to prepare and implement effective nursing care and develop solutions to problems concerning patient's mental or physical well-being. Ability to concentrate and pay close attention to detail for up to eighty percent of work time while: planning and performing professional nursing care, resolving patient care problems and dealing with patients, their families, and other professionals involved in the patients care, often during stressful situations.
SPECIAL SKILLS PREFERRED: None. LICENSURE DESCRIPTION: Requires current state of Wisconsin Registered Nurse License or a Multi-state Nursing License from a participating state in the NLC (Nurse Licensure Compact). LICENSURE PREFERRED: Certified Clinical Transplant Coordinator (CCTC) certification, Certified Clinical Transplant Nurse (CCTN), or Acute/Critical Care Nurse (CCRN) is preferred.
