Job Description

To provide assistance and treatment to program participants on CBRF (Community Based Residential Facility).

To help ensure active participation in the rehabilitation program.

To participate as a member of the interdisciplinary treatment team.

To provide education, assistance, supervision, safety, and behavior management for the program participant population.

Key Responsibilities:

1. Skill, Knowledge and Abilities: Knowledge of the sequela of brain injury and other neurological disorders. Knowledge of physical, cognitive, psychological,

emotional, and/or communication disabilities. Ability to obtain and maintain Mandt certification. First Aid course required.

2. Education: Preferred: High school diploma or equivalent may include completion of certified nursing assistant course with certificate. Certified as CNA if CNA.

3. Licensure: Valid driver's license and good driving record preferred

4. Experience: Preferred one year experience working within residential based or behavioral program


Qualifications:

1. Functions at all times as a therapeutic agent by demonstrating teaching, reinforcing, anticipating, interacting, and noticing program participant behavior.

a. Teaching- Looking for and using opportunities throughout/beyond activity to instruct, coach, support, and develop skills.

b. Reinforcing- acknowledging/encouraging target behaviors; providing regular/sincere praise and appropriate reinforcement.

c. Anticipating- expecting and managing antecedents: setting up to succeed; intervening early to avoid problem behaviors.

d. Interacting- focusing and engaging with participants; moving, communicating, and demonstrating active involvement.

e. Noticing- prompting/encouraging adaptive behavior, providing choices, attending, and observing participant/situation.


2. Tends to the program participant immediate needs for safety and performs all aspects of participant care in an environment that optimizes patient safety

and reduces the likelihood of medical/health care errors.

a. Demonstrates competency in implementing all aspects of the facility crisis intervention program, including safely managing resident at least restrictive level

of intervention. Maintains proper level of supervision.

b. Implements special treatment procedures according to facility policy. Demonstrates understanding of suicide/high risk precautions, 1:1 expectations.

3. Provides treatment in accordance with unit and facility policies through active participation with residents during activities, community outings,

ADL's, meals, groups, etc. in ways that foster independent functioning on the part of the participants.

a. Achieves 85%-95% "Meets Compliance Criteria" on Competency Assessment.

b. Reviewed all person centered plans, dietary, behavior, and medical protocols as evidence by signature on the review page. Effectively implements

interventions from protocols.

c. Engages participants in outings (monthly) and group activities (weekly). Completes shift duties in a timely manner. Seeks supervisor for additional tasks,

when assigned jobs are complete.

4. Actively and professionally participates in exchange of information. Interacts with family members, others in professional manner.

a. Demonstrates understanding of cultural/religious issues.

b. Demonstrates knowledge of resident grievance procedure and can assist with completion when requested.

c. Maintains close communication with supervisors and coworkers, has two way radio when available. Signs off on communication log at start of shift. Provides input

for shift change report.
