The second promise we make to our patients and families is they will not feel alone. What is Hospice Care? Hospice care is a specialized program of care provided for patients and their families who are facing a terminal illness. Seasons HospiceTeam provides expertise on pain and symptom management as well as spiritual and emotional support. Who is Seasons Hospice? Seasons Hospice is a community based organization on an ongoing mission to find creative solutions which add quality to life. We at Seasons Hospice hold steadfast to the patient/family focus of hospice care. We recognize that individuals and families are the experts in their own care. We are continuously striving for excellence beyond accepted standards. We work to increase the community’s awareness of hospice as part of the continuum of care. What Services are Provided? Who Pays for Hospice Care? Seasons Hospice bills directly to Medicare, Medicaid, and/or private insurance for hospice care. These benefits usually cover hospice care, including medications, supplies and equipment related to the terminal illness, at 100% with no cost to the patient. Hospice encourages referrals early on, so that the patient and family may receive maximum benefit from the comprehensive services that we provide. Hospice: Who Provides Care? Patients and families are the center of the palliative care team. It is around them that the whole team revolves. In order to provide the best possible care to patients and their families, the team typically includes the following members: |
